background:
Serum albumin (SA), also known as ALB, is the major protein of plasma and has a good binding capacity for water, Ca2+, Na+, K+, fatty acids, hormones, bilirubin and drugs. The main function of SA is to regulate the colloidal osmotic pressure of the blood. As the main zinc transporter in plasma, SA normally binds about 80% of plasma zinc. Variant structures of albumin may lead to increased zinc binding, resulting in asymptomatic increase in zinc concentration in the blood. Serum albumin defects can cause familial dysalbuminemic hyperthyroxinemia, a form of euthyroid hyperthyroxinemia caused by increased affinity of serum albumin for T4. It is the most common cause of hereditary euthyroid hyperthyroxinemia in the Caucasian population.
